In order to register for classes, all students must be in compliance with New York State law regarding immunizations.
1. Immunizations
Measles, mumps, and rubella
Immunization records must be provided to the Health Center. The Health Center can provide assistance for obtaining records and/or provide measles, mumps, and rubella immunizations at no charge. Please fill out the New Student Health Form here: New Student Health Form

2. Meningitis Information Response Form
You are not required to receive the meningocococcal vaccine, but you are required to fill out the Meningitis Response Form here: Submit your meningitis immunization information.
MANDATORY HEALTH INSURANCE POLICY
All full-time students are required to have health insurance.
Students registered for 12 credit hours or more, (undergraduate and graduate) are automatically enrolled into Buffalo State College's Health Insurance Plan. Students who have their own insurance are required to complete an online waiver to avoid being enrolled in the college's health insurance plan.
The deadline to waive out of the health insurance plan for Fall 2010 is September 30, 2010.
Requests to waive off of the school's insurance for Spring 2011 will be available in December 2010. Please fill out the waiver here: Online Health Insurance Waiver Form
