We encourage members of the campus community to complete the online reporting form for a student concern that needs immediate attention. Please remember that the University Police (716-878-6333) should be contacted in the event of an actual emergency occurring on campus.
All members of the campus community are invited to use this form for student concerns only.
Use of the form requires a campus logon and IP addresses will be recorded. This step is required to avoid abuse of the online reporting protocol.
Once the form is submitted, a report is generated to members of the Students of Concern Care Team, who will investigate the situation and obtain any additional information relevant to the report.
Please note that investigations are only conducted during regular business hours. If you believe your report requires immediate action, notify University Police at 716-878-6333.
Investigations may include a follow up phone call to the person making the report. Be aware that you may not be kept apprised of the outcome of an investigation due to confidentiality, but know that we greatly appreciate your assistance in keeping the campus community safe.
The online reporting form is solely to notify the Care Team that a student issue is a concern; this is not a forum for concerns relative to faculty or staff. Any submissions made regarding any faculty or staff concerns WILL NOT be acted upon by the team.
Any evidence of improperly using this form and/or fraudulent reporting is a violation of the Buffalo State code of student conduct and will result in appropriate disciplinary action.
