The online reporting form is available for the campus community to use. Please be advised that a campus logon is required. Only members of the campus community, faculty, staff and students will be able to use this form. I.P. addresses will be recorded, in addition to the logon information.

Please be sure to use proper Buffalo State logon format. Failure to use the proper format will prevent the online reporting form from being accessed.
